Patient Registration
You may pre register with our office by filling out the secure online Patient Registration Form below. After you have completed the form, please make sure to press the Submit button to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Reminder: Please bring a photo ID, medical and dental insurance cards, referral documentation, and all forms.

To help protect you from becoming a victim of identity theft, our practice will verify the identity of all new and returning patients of the practice.
Additionally, federal law requires all healthcare providers to obtain, verify and record information that identifies each new patient.
What this means for you: when you fill out your paperwork, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents.
Financial Information for Oral Surgery & Periodontics
Insurance
At Nu Image we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this, we work hand in hand with you and your insurance company to maximize your insurance reimbursement for covered procedures. Please bring your insurance information with you to your initial consultation so that we can expedite reimbursement. We accept most major insurance plans. Please call our office for your specific plan questions. If you have questions, please call (919) 841-1720 and ask to speak with our Insurance Coordinator.
Payment Terms
As a courtesy to our patients, we will file your insurance for you. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Your Initial Consult and Exam
At your consultation, one of our surgeons will evaluate your condition, determine your diagnosis and then explain your treatment options to you. On the day of your initial consultation, you will need to bring the following items:
- Your ID. Identification will be requested at the first visit.
- Your referral slip (if you have been referred to our office by a general dentist or other specialist).
- X-rays. Any that have been taken within the past year.
- A list of any medications that you are currently taking.
- Your insurance information including dental and medical insurance cards & necessary forms.

All patients under 18 years of age must be accompanied by a parent or legal guardian at their initial visit.
